Please use this form to describe any verbal warnings given to an employee. This will demonstrate that efforts to correct disruptive behavior were implemented before issuing a written warning.
Please use this form to request a formal written warning from the HR department. Ideally, this request is to be used once a verbal warning has been given and documented on the verbal warning form.
This form is to be used for termination requests. If the termination is due to anything other than “reduction in force”, detailed information will be gathered regarding previous warnings and attempts to correct the behavior. Even though Utah is an at-will state, we want to make sure we are documenting all infractions before terminating.
