I hereby authorize MCON | Southam | Uneeke | Ram | Fluid Dynamics to issue a company credit card(s) in my name for business-related expenses. I understand and agree to the following terms and conditions: Use of the Company Credit Card: I acknowledge that the company credit card is for official business expenses only and should not be used for personal transactions. Responsibility for Charges: I understand that I am responsible for all charges made on the company credit card issued in my name. I agree to use the card responsibly and in accordance with the company's expense policy. Disputed Charges: In the event of any disputed charges on the credit card statement, I will promptly notify the company's accounting department and cooperate in resolving the dispute. Card Protection: I will take all necessary precautions to protect the company credit card from loss, theft, or unauthorized use. If the card is lost or stolen, I will immediately report it to the appropriate company representative. Documentation and Expense Reports: I will maintain accurate records of all expenses charged to the company credit card and submit expense reports in a timely manner as per the company's policy. Any expenditure that is not shown on the expense report AND/OR not accompanied by a detailed receipt will be deemed a personal expense. A declaration detailing the nature of the expense and that it is business related may replace a detailed receipt if such receipt is not available. Otherwise, the expense will be deemed personal and deducted from the next paycheck. Cash advances on credit cards are not allowed. Termination of Employment: In the event of termination or separation from any of the MCON companies, I will return the company credit card and settle any outstanding expenses or balances as directed by the company.
